PARENTAL AUTHORIZATION AND RELEASE FORM
This Parental Authorization and Release Form is designed to facilitate the creation and distribution of the book “Young Maestros Compose” by Brian Hughes.
BACKGROUND:
Brian Hughes, a piano teacher and composer, is planning to publish a book featuring compositions developed by piano students, including your child, in both physical print and digital formats. This initiative aims to encourage a holistic approach to musical education, promoting the composition of original music as a means to enhance understanding of the musical and pianistic language. The book will be illustrated and is scheduled for publication at the end of 2024. A fundraising campaign has been launched on Brian Hughes' website to cover printing, digital art, and distribution costs. At the successful completion of the fundraising campaign a complimentary copy of the book with personal dedication will be provided to each child. Additionally, 10% of the income from book sales will be donated to a local charity supporting less privileged children pursuing a musical education.
AUTHORIZATION:
By signing this Authorization, the undersigned Parent(s) or legal guardian(s) grant Brian Hughes permission to include compositions created by their child in the aforementioned book, both in physical and digital formats. This includes the use of the child's name, and any accompanying information provided by Brian Hughes.
GDPR COMPLIANCE:
In accordance with the General Data Protection Regulation (GDPR), Brian Hughes ensures the following:
The personal data collected will be limited to the necessary information for the book publication, including the child's name, and any additional information provided voluntarily by the Parents. Should the legal guardians prefer the child name may be represented by initials only.
The personal data will be processed for the sole purpose of publishing the book and will not be used for any other purposes without explicit consent.
Brian Hughes will take reasonable steps to ensure the security and confidentiality of the personal data collected.
Parents have the right to access, rectify, or erase their child's personal data by contacting Brian Hughes through the contact details provided.
UK LAW SUITABILITY:
This Authorization is governed by the laws of the United Kingdom. Any disputes arising out of or in connection with this Authorization shall be resolved in accordance with UK law.
RELEASE AND INDEMNITY:
Parents hereby release and indemnify Brian Hughes from any claims, liabilities, or demands arising out of the publication of their child's composition in the book, including but not limited to claims related to privacy, defamation, or copyright.
SIGNATURES:
Parent(s) or legal guardian(s) hereby acknowledge that they have read and understood the terms of this Authorization and voluntarily consent to the publication of their child's composition in Brian Hughes' book.
